Can GLP-1s Raise Testosterone? What Men Taking Ozempic, Wegovy or Zepbound Should Know

GLP-1 medications have become some of the most talked-about drugs in men’s health. You’ve probably heard about Ozempic, Wegovy and Zepbound for weight loss, but lately, another question has been popping up:

Can GLP-1 medications increase testosterone, too?

The answer is more interesting than a simple yes or no.

Emerging research suggests that some men, particularly those with obesity and low testosterone, may see their testosterone levels improve while taking a GLP-1 medication. But that doesn’t make Ozempic or Zepbound a substitute for testosterone therapy, nor does it mean every man taking a GLP-1 will suddenly see his testosterone skyrocket.

Here’s what we know so far.

First, What Do GLP-1 Medications Actually Do?

GLP-1 medications help regulate appetite, blood sugar and digestion. Semaglutide is the active ingredient in Ozempic and Wegovy, while tirzepatide, the active ingredient in Zepbound and Mounjaro, targets both GLP-1 and GIP receptors.

One of their best-known effects is significant weight loss.

And that’s where testosterone enters the conversation.

The Connection Between Weight and Low Testosterone

Testosterone and metabolic health are closely connected.

Men with obesity are more likely to have lower testosterone levels, and excess body fat can interfere with the body’s hormonal signaling. The relationship can become something of a vicious cycle: weight gain may contribute to lower testosterone, while symptoms associated with low testosterone can make maintaining muscle mass, energy and an active lifestyle more challenging.

That’s important because losing excess weight can sometimes improve testosterone levels without testosterone replacement therapy (TRT).

In July 2026, the Endocrine Society reaffirmed that for appropriately diagnosed men whose hypogonadism is associated with being overweight or obese and who have no other identified cause, weight loss is typically considered first-line therapy.

So, Can GLP-1s Increase Testosterone?

Potentially—but context matters.

A recent systematic review found that GLP-1 receptor agonists may improve testosterone levels in men with metabolic problems, including obesity and hypogonadism. Some studies have also reported improvements in certain semen parameters, although researchers emphasize that more long-term controlled research is needed.

Research presented at the Endocrine Society’s ENDO 2026 meeting similarly suggested that GLP-1 medications may improve testosterone levels and sperm quality in men with obesity-related low testosterone.

Here’s the important part:

That doesn’t necessarily mean the medication itself is directly “boosting” testosterone.

Improvements in weight, insulin sensitivity and overall metabolic health may help the body’s existing hormonal system function more effectively.

In other words, the GLP-1 may be helping address one of the underlying factors contributing to low testosterone rather than acting as testosterone itself.

GLP-1s vs. TRT: They’re Not the Same Thing

Seeing your testosterone improve after losing weight on a GLP-1 is very different from receiving testosterone replacement therapy.

TRT replaces testosterone in men who have been appropriately diagnosed with testosterone deficiency. And a diagnosis isn’t based on feeling tired one afternoon or seeing one questionable lab result.

According to the Endocrine Society, diagnosing hypogonadism requires both symptoms consistent with testosterone deficiency and consistently low testosterone levels measured accurately through blood testing.

Symptoms worth discussing with a men’s health specialist may include:

  • Low sex drive
  • Erectile or sexual changes
  • Loss of muscle mass or strength
  • Low energy
  • Changes in mood or motivation
  • Difficulty concentrating
  • Fertility concerns

These symptoms can have many causes, which is exactly why testing and a comprehensive evaluation matter.

What About Fertility?

This is another area where things get interesting.

Traditional testosterone therapy can suppress the body’s production of sperm, which makes fertility an important consideration for men considering TRT.

Early research suggests GLP-1 medications may offer a different picture. A 2026 systematic review found potential improvements in testosterone and semen quality among some men with metabolic dysfunction while maintaining the hormonal signals involved in testicular function. Researchers have even suggested GLP-1s warrant further study as a possible fertility-preserving approach in certain cases of obesity-related low testosterone.

That’s promising, but it’s still emerging research, not a reason to self-treat infertility with a GLP-1.

Don’t Forget About Muscle

There’s one more piece of the GLP-1/testosterone conversation that men shouldn’t overlook: muscle mass.

Rapid or significant weight loss can include loss of lean mass along with fat. Research published in 2026 found that semaglutide and tirzepatide preferentially reduce fat mass, but some reduction in lean mass can occur as well.

That’s why the goal shouldn’t simply be to make the number on the scale as small as possible.

Adequate nutrition, sufficient protein, resistance training and monitoring body composition can all become important parts of maintaining strength and muscle during weight loss.

The Bottom Line

GLP-1 medications aren’t testosterone boosters, but for some men, improving weight and metabolic health may also help improve testosterone levels.

And that’s a good reminder that testosterone doesn’t exist in a vacuum.

Weight, sleep, medications, metabolic health, exercise, stress and other medical conditions can all play a role in how you feel and how your hormonal system functions.
