Testosterone vs. Sildenafil-Based Treatments for Female Sexual Dysfunction

Let’s talk about something that doesn’t get nearly enough airtime: your sex life. If desire has gone quiet, arousal feels like it’s stuck in traffic, or the spark just isn’t sparking, you are not broken and you are definitely not alone. Female sexual dysfunction (FSD) is incredibly common, and the good news is that treatment options exist. Two of the most talked about? Testosterone and sildenafil (yes, the “little blue pill” you thought was just a guy thing). Let’s break down how they stack up.

First, What Are We Actually Treating?

FSD is an umbrella term. Under it live things like low libido in women (clinically called hypoactive sexual desire disorder, or HSDD), trouble with arousal, and difficulty reaching orgasm. Desire and arousal are wired differently, one is about wanting, other is about your body physically responding, and that distinction matters a lot when choosing treatment.

Team Testosterone: The Desire Booster

When the issue is wanting sex, testosterone is the frontrunner. It’s the most evidence backed option for HSDD in postmenopausal women, endorsed by a global panel of experts and major medical societies as a moderately effective treatment (Davis et al., Climacteric, 2019).

Here’s the real talk version:

  • It works, modestly. Transdermal (through the skin) testosterone reliably improves sexual desire, arousal, orgasm, pleasure, and the number of satisfying sexual encounters, while also lowering the distress that comes with it. Think roughly one extra satisfying sexual event per month on average (Davis, New England Journal of Medicine, 2024).
  • It’s off-label. There’s currently no FDA-approved female testosterone product in the U.S., so clinicians carefully use male formulations in tiny, female sized doses with regular blood monitoring (Davis et al., Climacteric, 2019).
  • Side effects are usually mild, think a little acne or extra hair growth. Serious risks weren’t seen in studies of physiologic doses, though long term safety data are still catching up (Davis, New England Journal of Medicine, 2024).

The catch: the strongest evidence is in postmenopausal women. In premenopausal women, the data are promising but thinner.

Team Sildenafil: The Blood Flow Player

Sildenafil works on plumbing, not passion. It boosts blood flow to the clitoris and vaginal tissue, which theoretically helps with arousal, not desire. So it’s a different tool for a different job.

The honest scorecard:

  • Oral sildenafil has been a letdown for most women. Large trials showed it doesn’t reliably beat placebo for general FSD, so it isn’t recommended outside of specific situations (Davis, New England Journal of Medicine, 2024).
  • Where it may shine: women with arousal problems tied to antidepressants (SSRIs) have seen real benefit, and there’s emerging interest in specific arousal disorders (Reisman et al., Nature Reviews Urology, 2026).
  • The new frontier, topical sildenafil cream. A 2024 randomized trial found that a sildenafil cream improved arousal sensation, desire, and orgasm (and reduced distress) in a subset of premenopausal women with female sexual arousal disorder, with few side effects (Johnson et al., Obstetrics & Gynecology, 2024). Still investigational, but exciting.

So…Which One Wins?

Trick question, they’re not really competitors. They target different problems:

If your main issue is… The better-studied option is…
Low desire (not wanting sex) Testosterone
Arousal (body not responding) Sildenafil (especially SSRI-related; cream is emerging)

And here’s a fun plot twist: researchers are even combining the two. Early studies of testosterone plus sildenafil, timed together, show promise for certain women whose brains are less responsive to sexual cues (Reisman et al., Nature Reviews Urology, 2026). Science is officially getting creative.
