The Perimenopause Decade: What’s Actually Happening to Your Body in Your 40s

You’re 43. You haven’t slept properly in six months. You cried at a paper towel commercial. Your period showed up twice in three weeks and then ghosted you for forty days. And you’re pretty sure you left your keys in the refrigerator.

Your doctor says your labs look “normal.”

Welcome to perimenopause, the least-discussed, most-misdiagnosed decade in women’s health.

At Maze Women’s Health, we hear a version of this story every single week. So let’s clear a few things up.

Perimenopause Is a Transition, Not a Switch

Menopause is a single day: 12 months after your last period. Perimenopause is everything leading up to it, and it can last more than a decade.

During this stretch, your ovaries aren’t quietly winding down. They’re doing something much more chaotic: wide hormonal fluctuations, altered feedback signaling, and increasingly irregular cycles.

That’s the key insight most women never get. Perimenopause isn’t “low estrogen.” It’s unpredictable estrogen, which is exactly why the symptoms feel so random.

Most women start noticing changes in their 40s. Some in their late 30s. And a majority of women will experience symptoms during this transition.

The Symptoms Nobody Warned You About

Yes, hot flashes. But the list is much longer, and much weirder, than anyone told you:

  • Sleep disruption — waking at 3 a.m. for no reason, night sweats

  • Mood changes — irritability, anxiety, low mood that doesn’t match your life

  • Brain fog and temporary cognitive changes — the word-finding thing is real, and it’s usually temporary

  • Vaginal dryness and urinary changes — collectively called genitourinary symptoms, and unlike hot flashes, these tend to get worse over time rather than better

  • Low sexual desire

  • Increasing abdominal fat and metabolic shifts

  • Bone loss — silent, and starting now

Here’s a genuinely useful data point: in an analysis of over 147,000 symptom logs from nearly 4,800 women, the symptoms most commonly reported across all reproductive stages were fatigue, headache, anxiety, and brain fog, and hot flashes turned out to be a poor predictor of any other symptom except night sweats.

Translation: you can be deep in perimenopause without a single hot flash.

Perimenopausal women in that dataset were distinctive because they reported both menstrual-cycle-type symptoms and vasomotor symptoms simultaneously. Which is exactly why it feels like your body can’t pick a lane.

Why “Your Labs Are Normal” Isn’t the Answer

Because during perimenopause, hormone levels swing wildly, a single blood draw is a snapshot of a moving target.

Perimenopause is primarily a clinical diagnosis, based on your age, your cycle pattern, and your symptoms. Not one FSH level on a random Tuesday.

If you’ve been told “you’re too young” or “your labs are fine,” you deserve a second conversation.

What Actually Helps

Good news: there’s a lot, and it’s not all or nothing.

Hormone therapy remains the most effective treatment for vasomotor symptoms (hot flashes, night sweats) and genitourinary symptoms. For women under 60 and within 10 years of menopause onset, and without contraindications, the benefit-to-risk balance is generally favorable.

Important nuance: much of the fear around hormone therapy came from studies in women who were over 60 or more than 10 years past menopause, a very different risk profile than a healthy 47-year-old. Timing matters enormously.

Non-hormonal options exist too, and they’re legitimate, not consolation prizes.

Lifestyle strategies with real evidence:

  • Resistance training — supports bone, muscle, mood, and metabolic health

  • Yoga and mind-body practices — improve physical and psychological well-being

  • Plant-forward diets and phytoestrogens — may help vasomotor symptoms while supporting heart and bone health

  • Calcium, vitamin D, magnesium, and vitamin K2 — support bone health during a period of accelerating bone loss

The Bone Thing (Please Don’t Skip This Part)

Here’s the part that gets buried under the hot flash conversation.

The years around menopause are when bone loss accelerates, quietly, painlessly, and without symptoms until something breaks. Osteoporosis is diagnosed by a T-score of −2.5 or lower on a DEXA scan, and postmenopausal women at average risk are screened starting at 65.
